The following details are in accordance with the General Data Protection Regulation. By giving your consent to Dudley Carers Forum (DCF), you agree that your details can be held and used in the following way.
DCF asks only for your name and either email address or home address, so that we can contact you with details of the Forums activities and for no other purpose. This is our legal base.
Your details are kept on a computer database and encrypted for security. The computer is used only for DCF.
DCF will never share or sell your information to anyone else unless, you ask us to do so.
You can request to be removed* from the database, or for details to be changed* at any time. DCF will do this within one month.
You can ask DCF what details they have recorded about you*, this will be done within one month.
If the database on which your details are kept has a security breach, a report will be made to the supervisory authority ICO within 72 hours.
Only elected Officers of DCF will be able to access your information.
Your details will be kept until such a time that you ask for them to be removed.

If you think your data has been misused or that DCF hasn’t kept it secure, you should contact us and tell us.
If you’re unhappy with our response or if you need any advice you should contact the Information Commissioner’s Office (ICO).

If you require DCF’s support in another matter your consent must be gained separately. Additional information given will only be kept until the matter is resolved. DCF may want to anonymise the case to use at a later date. Your consent will be required to do this.

Membership to Dudley Carers Forum is open to Unpaid carers and ex carers of Dudley MBC and surrounding area. If you do not qualify on these grounds but would like to receive information please mark your application as 'legitimate business interest' Thank you.
